REQUEST A NEW PRESCRIPTION

If you are an existing patient at The Carriage House Clinic and would like to request a new private prescription from your doctor please use the link below. Private prescriptions incur a £35 administration fee and you will receive an invoice once your request has been completed. Private prescriptions are not repeats and you need to request a new prescription before placing your order with either The Independent Pharmacy or Pharmacierge.

Please note prescription requests may take up to three working days to be processed.
