The Carriage House Clinic Private GP service and Oxford Menopause are both provided by Oxford Menopause Ltd and regulated by the Care Quality Commission (CQC).

  1. Members are patients who pay a monthly or annual subscription to the practice.

  2. Becoming a member

    1. Patients must complete an online application form to apply for membership.

    2. The Carriage House Clinic has the right to decline a membership application for any reason. This could include if we feel the patient has a complex medical history or condition that would be better managed by another healthcare professional.

    3. If we decline your application, we will refund any membership fees you have already paid.

    4. Our Service will commence when we have accepted your application and received your payment.

  3. Fees

    1. Full details of our membership fees are available on our website.

    2. The subscription rates are reviewed periodically, and members will be informed in writing of any changes before they are implemented.

    3. Membership fees will not be increased within the first 12 months of your membership.

  4. Membership Services

    1. Included in the membership fee is an annual health check which is provided free of charge including diabetes and cholesterol blood tests.

    2. Members pay 50% of the advertised price for GP appointment fees. They will still be liable for late cancellation fees at this reduced rate.

    3. Fair usage: Where we (in our reasonable opinion) believe that your use of the Services in any month is excessive, we shall notify you of the same and we may amend your membership fee structure accordingly to reflect your usage of the Services.

    4. Included in the membership is the writing of private prescriptions and referral letters issued at the time of your consultation. The costs of medication provided by a pharmacy will still be payable by the patient.

    5. Prescriptions and referral letters issued other than at the time of a consultation will still be charged at the normal rate.

    6. Any additional services (including non-GP appointments, investigations and procedures) will be charged at full cost. You will be informed of any additional costs prior to incurring the same. 

  5. Membership cancellation

    1. Within the first 14 days of your membership, you may cancel your agreement with us, providing you have not used any of our services, and we will refund your fees paid within 14 days from the day on which we receive your cancellation notice. 

    2. Subject to 5.1 above, membership payments may not be cancelled within the first 12 months.

    3. After the first 12 months, you may cancel your membership by notifying us in writing.

    4. If a member paying annually cancels their membership they will be refunded pro-rata for complete months still left on the membership.

    5. We may cancel our membership scheme at any time.

    6. In the event of continued inappropriate usage of our Service by an individual, we reserve the right to immediately cancel membership benefits in order to protect the care provided to all other Members.
